Established in 1999 in IN from the proceeds of the sale of Kosciusko Community Hospital to Quorum Health Group, Inc

Purpose and Activities

The foundation exists for the benefit of Kosciusko County, Indiana citizens to ensure healthcare services are provided, and to advance prevention and healthy lifestyles. This will be accomplished by identifying health needs in our community and maintaining an endowment so funding is available, through investments and grants, for those needs.

Cancer Care Fund Program

This program provides assistance to financially-eligible residents of Kosciusko County, IN, who have been diagnosed with cancer within the last three months. The purpose of the fund is to relieve some of the financial strain that often accompanies the diagnosis. The assistance provided includes, but is not limited to items such as rent or mortgage payments, utilities, insurance, food, car payments, and prescription medications. Grants to Individuals Program

Scholarships to financially needy residents of Kosciusko County, IN, who are pursuing a degree in health-related professions pertaining to direct patient care, as well as assistance to financially needy individuals who have had a diagnosis of cancer within the last three months, and reside in the Kosciusko County, IN, area.

Program-Related Investment

The foundation has made a PRI in the form of a loan to a health services organization, as well as to a food services organization.
