Purpose and Activities

Dedicated to helping metropolitan Detroit organizations meet the community's health care needs. Grants support a wide variety of health-related services for people of all ages. Collaborative programs which make comprehensive, primary health care accessible to at-risk urban groups are a priority. Areas of primary interest include: teenage pregnancy crisis; health care transportation for the elderly and homeless; in-home healthcare assistance; medically-related assistance to the homeless; healthcare equipment and medical supplies in limited amounts, preferably for underfunded organizations; medical education in special situations in the tri-county Detroit, Michigan, area; and start-up and support money for projects such as AIDS, epilepsy, cancer, heart disease, leukemia, and other major ills of substantial public concern. Support given to all medically indigent people.

Incorporated in 1986 in MI; converted from Group Health Plan of Southeast Michigan - The foundation was formed in 1986 through the sale of a consumer-directed, nonprofit health maintenance organization to a for-profit hospital consortium. In the early 1970's, the Black and Polish-American Conference organized by U.S. Rep. John Conyers, communicated various needs within the Detroit and Hamtramck community. According to the conference, the community's most pressing needs were the establishment of community-focused health programs and improved access to primary care. Approximately 10 years later, the consortium opened its doors as an HMO with the intent to provide services to meet those needs. Notwithstanding the consortium's work, it took the subsequent sale of the HMO and the establishment of the foundation to begin supporting what the community said it wanted and needed
